[Mycroft switches to video, clearly John had amnesia, and figured perhaps a familiar face might trigger a memory.
He sounds actually relieved, but also...empty and tired, like he'd been through Too Much. It was the face of a man who's had his worst fear come to life recently. He can sympathize with needing this to be a dream. It still felt like one. A part of him clung stubbornly to it.
But despite all the horror and madness, he had not yet waken up. Dream was looking less and less likely.
There's makeshift bandages on his knuckles from an old handkerchief, stained with a small amount of day-old blood, and fairly fresh bruising along the side of his jaw and nose. He'd stayed in the building for too long and had apparently fought his way through the cast of Les Mis (AKA several doors), the bruise came from the hallucinations causing him to misjudge where he was and running headlong into a wall. Fortunately he regained his senses once he'd managed to get outside.]
Tell me what the exact last thing you remember is.

May I inquire as to your area of medical expertise? It would be helpful to know what you're most skilled with so that we can be sure when it's best to approach you. There are a few other doctors here, so you needn't take full responsibility for offering medical advice.

I completed my residency in A&E and have performed as a battlefield medic and trauma surgeon. I'm also licensed as a general practitioner, if that's going to come in a bit handier.

Whatever happened between your last memory of home and your arrival here I think must have brought you some peace. You seem guarded, wary of attack from anywhere and anyone.
Whether or not you believe us about your memories, we are all prisoners here. Most of us would rather cooperate than fight.
